We recommend that you provide the following:

  • Photo ID
  • Auto insurance information.
  • A referral or authorization number (if required for the visit).
  • A current list of your medications including over-the-counter meds and nutritional supplements.
  • A list of prior medical and surgical procedures.
    • If your chief complaint involves cognitive or memory issues, we advise you to be accompanied by a family member or caregiver who can assist the medical team with background information on your condition.
  • You should plan on arriving 10 to 15 minutes ahead of your scheduled visit to ensure that all the required paperwork is in order.

Yes, typically the testing is done in-clinic unless additional testing at an imaging center is required. Also, if you show up late to your appointment, we cannot guarantee that the diagnostic testing can be fully completed at this visit. We schedule our visits in block time to ensure we have appropriate time with each of our patients.

In the event you need to cancel/reschedule your appointment, you should call our office during regular business hours at least 24 hours prior to your scheduled appointment.

We don’t recommend rescheduling because when a possible TBI has occurred the sooner that the diagnostics can be completed the sooner the customized treatment interventions can take place.
